To register your child:

 

1. For ALL athletes- fill out the Universal Registration Web Form HERE.

2. Download and sign the Athlete Guidelines & Parent Guidelines Form.

 

3. Download and fill out the Ignite Sports Physical Form.

ALL completed physical forms are to be submitted by EMAIL ONLY.

Send completed forms with a PDF or JPG file attached to:   physicals@igniteathletics.org
 

4. Download and read the Ignite Homeschool Sports Handbook.
